Alzheimer's and Dementia Care

At The Phoenix Center for Rehabilitation and Pediatrics, our Alzheimer's and Dementia Care Program is renowned for its compassionate and comprehensive approach to care. We understand the unique challenges faced by individuals with dementia, and we are dedicated to providing a nurturing environment that promotes comfort, cognitive stimulation, and meaningful social interactions.

Tailored Care Plans

Our experienced team conducts thorough evaluations of each resident to develop personalized care plans that address the specific needs and abilities of individuals with dementia. Recognizing the diverse nature of dementia symptoms, our approach is flexible and adaptive, ensuring the highest quality of life possible for our residents.

Encouraging Socialization and Engagement

Research has shown that social interaction can significantly impact the progression of dementia, helping to preserve cognitive functions for a longer period. Our program fosters a vibrant community atmosphere where residents can engage in group activities, therapeutic exercises, and social events designed to maintain their social skills and encourage positive interactions.

Understanding Dementia

Dementia encompasses a range of symptoms associated with declining brain function, including memory loss, communication difficulties, impaired reasoning, and challenges in managing daily activities. Our staff is specially trained to support residents through these changes, offering compassionate care and understanding at every stage of their journey.

Comprehensive Support Services

We provide an array of services tailored to the needs of individuals with Alzheimer’s and dementia, including cognitive therapies, physical and occupational therapy, and nutritional support. Our secure, welcoming environment is designed to minimize confusion and maximize residents' independence and dignity.

At The Phoenix Center, we are committed to setting the standard in Alzheimer’s and Dementia Care. Our goal is not only to slow the progression of dementia but also to enrich the lives of our residents, providing them and their families with peace of mind and a sense of community during their time with us. Join us in creating a supportive, engaging, and caring environment for your loved one.

A CMS 5-Star Rated Facility in Quality Measures

Ratings for the quality measures are based on performance on 15 of the QMs that are currently posted on the Care Compare website. These include nine long-stay measures and six short-stay measures. Note that not all the quality measures that are reported on Care Compare are included in the rating calculations. In addition to an overall quality measure rating, separate QM ratings for short-stay measures and long-stay measures are also reported.
