Our Pediatric Chronic Illness Management Program is dedicated to providing comprehensive, individualized care to children and adolescents up to age 21, aiming to enhance their quality of life and promote overall well-being.​

Our Individualized Approach
Each participant in our program undergoes a comprehensive evaluation by our team of medical providers and mental health professionals. This assessment examines the patient's physical, emotional, mental, and psychological status, allowing us to develop a personalized treatment plan. Recommendations may include ongoing outpatient services, referrals to specialty providers, mental health services, and/or admission to our structured inpatient program.
