Chronic Illness

At the Phoenix Center for Rehabilitation and Pediatrics in Haskell, NJ, we understand the complexities and challenges that come with managing chronic illnesses in children. Our Pediatric Chronic Illness Management Program is dedicated to providing comprehensive, individualized care to children and adolescents up to age 21, aiming to enhance their quality of life and promote overall well-being.​

Our Specialized Services

Our program focuses on treating a wide range of chronic, life-altering conditions, including but not limited to:

  • Asthma

  • Diabetes

  • Cystic fibrosis

  • Sickle cell disease

  • Heart disease

  • Autoimmune disorders

  • Metabolic disorders

  • Organ transplants

  • Chronic respiratory diseases

  • Other conditions requiring strict adherence to treatment regimens

Our Individualized Approach

Each participant in our program undergoes a comprehensive evaluation by our team of medical providers and mental health professionals. This assessment examines the patient's physical, emotional, mental, and psychological status, allowing us to develop a personalized treatment plan. Recommendations may include ongoing outpatient services, referrals to specialty providers, mental health services, and/or admission to our structured inpatient program.

 

Behavioral and Therapeutic Interventions

Our approach incorporates various behavioral and therapeutic interventions to support skill development and health management, such as:

 

  • Direct instruction to develop new skills and knowledge

  • Shaping and reinforcement of appropriate behaviors

  • Experiential learning and generalization of skills across activities and settings

  • Use of visual tools to aid understanding

  • Medication education

  • Structured nutritional counseling and instruction

  • Supportive psychological counseling

Comprehensive Treatment Environment

Our facility offers a nurturing environment designed to promote healing and development through:

  • Intensive and varied daily exercise programs

  • Shopping and cooking experiences to encourage good nutrition

  • Multiple daily therapies and activities

  • Outings to facilitate community reintegration

  • Family education and counseling sessions

  • Psychological and educational testing, as needed, to identify learning issues

Outpatient Support Services

Beyond our inpatient program, we offer outpatient services that include individual or group mental health support, nutrition counseling, and physical and/or occupational therapy evaluations and treatments. Our goal is to provide continuous support tailored to each child's evolving needs.​

Family-Centered Care

We recognize the vital role families play in managing chronic illnesses. Our team provides families with the information and support necessary to understand their child's condition, treatment plan, and care requirements at home. We are committed to empowering families through education and collaborative care.​

A CMS 5-Star Rated Facility in Quality Measures

Ratings for the quality measures are based on performance on 15 of the QMs that are currently posted on the Care Compare website. These include nine long-stay measures and six short-stay measures. Note that not all the quality measures that are reported on Care Compare are included in the rating calculations. In addition to an overall quality measure rating, separate QM ratings for short-stay measures and long-stay measures are also reported.
