The moment a child enters the Children’s Center at The Phoenix, they become part of our family. The children and family member’s needs are dynamic and change as each child grows and develops. We are dedicated to ensuring that we meet each and every one of those needs.

The Bridgeway Program at the Phoenix provides a person-centered approach with various services for our young adult residents. Our full-service program focuses on the mind, body, and spirit in a  comfortable and safe environment.

A CMS 5-Star Rated Facility in Quality Measures

Ratings for the quality measures are based on performance on 15 of the QMs that are currently posted on the Care Compare website. These include nine long-stay measures and six short-stay measures. Note that not all the quality measures that are reported on Care Compare are included in the rating calculations. In addition to an overall quality measure rating, separate QM ratings for short-stay measures and long-stay measures are also reported.
